What needs a closer look?
Suppose you like the sound of the benefits. Before you decide, what belongs in the conversation?
Talk about unwanted effects, your own health history, and who will answer questions after treatment starts.
Stomach problems are common
In Zepbound (tirzepatide) weight trials, about 25–29 in 100 people on the drug had nausea, meaning they felt sick to their stomach. In the placebo group, which had a dummy treatment, 8 in 100 felt that way.
Other common problems in those trials were diarrhea, vomiting, and constipation.
The label describes severe stomach problems and kidney injury linked to fluid loss from vomiting or diarrhea.
The label also reports inflammation of the pancreas, meaning swelling and irritation of that organ. It helps the body handle blood sugar. Signs of this problem include severe belly pain or pain that does not go away, sometimes reaching the back. The label also reports gallbladder trouble and serious allergic reactions.
Ask which symptoms need a prompt call, and whom to call. Get that plan before leaving the visit.
What the thyroid caution actually says
The small organ called the thyroid sits in the neck.
The Food and Drug Administration (FDA) labels for tirzepatide have a boxed warning, the agency’s strongest warning format.
Rats developed thyroid tumors in the studies. It is unknown whether people can develop such tumors because of tirzepatide.
A rare cancer in the thyroid is called medullary thyroid cancer. The labels rule out tirzepatide for anyone with a personal or family history of that cancer. They also say not to use it with MEN 2, a specific rare illness that a parent can pass on to a child.
If you only know that someone in your family had thyroid cancer, tell the clinician. Ask them to check the medical records for the type of cancer and for MEN 2.
The labels also say not to use it if you have a known serious allergy to tirzepatide or another ingredient.
Bring up other medicines and pregnancy
Combining insulin or certain diabetes pills with tirzepatide can lead to blood sugar falling too low.
Starting Zepbound or increasing its dose may reduce how well birth control pills work, according to the label.
The label tells prescribers to discuss a non-pill or backup method for 4 weeks after each of those changes.
The label says Zepbound is to be stopped when pregnancy is recognized; pregnancy plans belong in a conversation with your clinician.
Before planned surgery, tell the team about tirzepatide. The labels warn that food still in the stomach can get into the lungs during surgery. This can happen under general anesthesia, which puts you to sleep, or deep sedation, which makes you very drowsy.
Ask the surgical team how this warning affects the plan for your procedure.
When allergy symptoms need urgent help
A severe allergy may show up as breathing trouble, difficulty swallowing, or a swollen tongue or face.
For severe allergy symptoms, the advice from MedlinePlus is to seek help at the closest emergency room. MedlinePlus is the government’s health information service.
This is a reason to get help straight away, not to wait for the next routine visit.
Pause at claims that do not add up
Under federal law checked September 26, 2026, a medicine requiring a prescription could not be dispensed unless that prescription was valid.
A pharmacist or doctor makes a compounded preparation to order. In the September 26, 2026 review, these preparations had no FDA approval.
The FDA also said on its guidance page, checked September 26, 2026, that claims of equal results between compounded and approved products were misleading.
A polished page cannot replace a check of the named clinician and pharmacy. Ask who made the product and how to reach someone after the visit.
